Entity Numbers for Non-Instructional Facilities

GUIDANCE FOR SUBMITTING REVISED ITEM 21 ATTACHMENTS

Revised Item 21 Attachments may be submitted by e-mail, fax or on paper.

To submit a Revised Item 21 Attachment:

  • Clearly indicate on the revised attachment(s) which non-instructional facilities are receiving services.
  • Label the revised attachment “REVISED ITEM 21 ATTACHMENT FOR FUNDING YEAR 2004” at the top of the page.
  • Include the following information at the top of the first page of the revised attachment:

    Form 471 Applicant’s Form Identifier
    Form 471 Application # (if available)
    Billed Entity Name
    Billed Entity Number
    Original Item 21 Attachment Number
    Contact Person’s Name
    Contact Person’s Telephone Number

  • Submit the Revised Item 21 Attachment:
